NPI Lookup

Search the National Provider Identifier (NPI) Registry to find publicly available information about healthcare providers and organizations in the United States.

Data Source: National Plan and Provider Enumeration System (NPPES (opens in a new tab)), Centers for Medicare & Medicaid Services.

NPI information is based on publicly available NPPES records. An NPI does not by itself confirm that a healthcare provider is currently licensed or credentialed.

What Is an NPI Number?

NPI stands for National Provider Identifier. It is a unique 10-digit number used to identify covered healthcare providers in the United States.

NPIs are issued through the National Plan and Provider Enumeration System (NPPES), run by the Centers for Medicare & Medicaid Services. Each NPI belongs to one provider and does not change, and an NPI lookup returns the public record NPPES holds for it.

  1. ProviderAn individual or organization
  2. NPIOne unique 10-digit number
  3. Provider RecordPublic NPPES details
Each provider has one NPI, and the NPI points to that provider's record in NPPES.

How to Use the NPI Lookup

  1. Enter an NPI number or provider information.

Use Search by NPI for a 10-digit number, or Search by Provider for a name, organization, specialty or location.

  2. Select Search.

The lookup queries the NPPES NPI Registry and returns matching records.

  3. Review the available NPPES provider information.

Open a result to see its details, taxonomy and practice addresses.

What Information Can You Find With an NPI Lookup?

An NPI record can include the details below. Not every record contains every field, and the lookup shows only what NPPES returns.

  • Provider name
  • NPI
  • Provider type
  • Taxonomy
  • Specialty and classification information, where available
  • Practice address
  • Business phone
  • Enumeration date
  • Record update information

NPI-1 vs. NPI-2

NPPES issues two types of NPI: one for individual providers and one for healthcare organizations.

NPI-1

Individual Healthcare Providers

Issued to a person who provides healthcare. Examples include:

NPI-2

Healthcare Organizations

Issued to an organization that provides healthcare. Examples include:

What Is an NPI Taxonomy Code?

A taxonomy code classifies a healthcare provider by provider type, classification and, where one applies, specialization. Each NPI record lists one or more taxonomy codes, and one of them is marked as primary.

In NPPES, a taxonomy may also be listed with the state and license number the provider reported alongside it.

  1. ProviderAn individual or organization
  2. Provider Typee.g. Allopathic & Osteopathic Physicians
  3. Classificatione.g. Internal Medicine
  4. Specializatione.g. Cardiovascular Disease
Example: taxonomy code 207RC0000X. Not every code has a specialization level.

Why Use an NPI Lookup?

  • Finding an NPI

Search by name and location when you do not have the number.

  • Checking publicly available provider information

See the name, type and status NPPES holds for an NPI.

  • Reviewing taxonomy information

See a provider's primary and additional taxonomy codes.

  • Reviewing practice information

See practice and mailing addresses and business phone numbers.

  • Researching healthcare organizations

Look up NPI-2 records for hospitals, clinics and groups.

  • Confirming information shown in NPPES

Compare what you have on file with the current registry record.

An NPI lookup shows what is recorded in NPPES. It does not verify a medical license.
